Referring a New Patient: To refer a new patient to our office call our main number at (719)471-9942 and ask to speak with new patient registration. Be sure to have the patients demographics available to schedule appointment.
	Medical Records: Electrophysiology Associates will need: Any recent rhythm strips and/or EKG's which document the patients abnormal heart rhythm Echocardiogram or other cardiology testing Physician notes.
Paperwork: Patient's must fill out paperwork before their scheduled consultation or tilt table test. If patient does not have paperwork filled out before initial appointment, please have patient arrive 20 - 30 minutes before scheduled appointment time.
Referrals: If patient's insurance requires referral: As the referring physician, be sure to supply a proper insurance carrier referral for office visit. Patient will not be seen without a referral in our office.
