Registration Information
When you call to make an appointment please let the receptionist know that you are a new patient. You will be asked to arrive 20 - 30 minutes early for your first appointment to complete the necessary paperwork. Be sure to bring your insurance card with you. |

Patient Privacy Information
The Federal Government has required that your medical records remain private, confidential, and absoulutely not available to anyone without your expressed written consent. Our medical record of your care remains the physical property of Electrophysiology Associates. The State of Colorado has supported this law. |
Insurance Information
Electrophysiology Associates accepts a wide variety of insurance carriers. But, many insurance carriers, primary/secondary, require a referral in order to see one of our specialists. We provide a complete list of carriers we accept, as well as information as to which insurance carriers require referrals. |
